Selective Bronchial Occlusion for Treatment of a Bronchopleural Fistula in an Extremely Preterm Infant
Giacomo Simeone Amelio, Mariarosa Colnaghi, Silvia Gulden, Genny Raffaeli, Valeria Cortesi, Ilaria Amodeo, Giacomo Cavallaro, Fabio Mosca, Stefano Ghirardello
Summary: Neonatal pulmonary air leak is a common complication of mechanical ventilation in infants with hyaline membrane disease. While conservative management or chest drainage can be effective, severe cases may require alternative therapeutic solutions. Selective bronchial occlusion, as described in the study, is a promising unconventional rescue therapy for bronchopleural fistula resistant to standard treatments, showing significant improvement in clinical condition and radiographic findings.

Bronchopleural Fistula in the Mechanically Ventilated Patient: A Concise Review
John C. Grotberg, Robert C. Hyzy, Jose De Cardenas, Ivan N. Co
Summary: The key to managing bronchopleural fistula in mechanically ventilated patients is to reduce air leak by strategies such as decreasing peak inspiratory pressure, using lower tidal volumes, lowering positive end-expiratory pressure, decreasing inspiratory time, and reducing respiratory rate. Individualized mechanical ventilation and intrapleural catheter management are important.

Secondary Pneumothorax Induced by a Bronchopleural Fistula in a Patient With COVID-19 Pneumonia
Connor Smith, Olivia Mobarakai, Amber Bux, Ralph Kamel, Neville Mobarakai
Summary: COVID-19, caused by SARS-CoV-2, is a multi-system disease characterized by pneumonia complicated by adult respiratory distress syndrome. While spontaneous pneumothorax has been reported in some COVID-19 patients, bronchopleural fistula has rarely been reported as the primary cause. We describe a rare case of a young patient who developed pneumothorax complicating COVID-19 and was found to have a bronchopleural fistula and empyema secondary to Staphylococcus aureus superinfection.

Maria Mohammed Fariduddin, Kai Wang, Ayesha Birjees, Wajihuddin Syed
Summary: The case involves a 19-year-old female presenting with pneumonia and sepsis, complicated by empyema and bronchopleural fistula. Bronchopleural fistula is a rare complication that requires surgical or bronchoscopic intervention and adjustment of ventilator settings to support respiratory function. Recognizing and managing a BPF promptly in the emergency setting is crucial for ensuring patients receive appropriate treatment for the fistula.

Zaryab Umar, Usman Ilyas, Salman Ashfaq, Deesha Shah, Mahmoud Nassar, Theo Trandafirescu
Summary: Bronchopleural fistulas (BPFs) are a serious complication associated with high morbidity and mortality in COVID-19 patients. This case report presents an 86-year-old male with COVID-19 pneumonia and subsequent BPF, who experienced persistent air leak. The use of endobronchial valves resulted in successful cessation of the air leak, providing guidance for managing this condition.

Summary: Bronchopleural fistulas are rare complications of bevacizumab treatment. This case report describes a patient who developed a bronchopleural fistula after undergoing bevacizumab therapy for lung cancer. Surgical repair of the fistula using muscle flaps resulted in satisfactory healing and the patient remained alive without recurrence for 5 years. Careful postoperative management is necessary when using bevacizumab for induction therapy.

High-resolution three-dimensional contrast-enhanced magnetic resonance venography in children: comparison of gadofosveset trisodium with ferumoxytol
Puja Shahrouki, Sarah N. Khan, Takegawa Yoshida, Paul J. Iskander, Shahnaz Ghahremani, J. Paul Finn
Summary: This study compared gadofosveset trisodium and ferumoxytol for high-resolution MR venography in children, finding that ferumoxytol had higher SNR and CNR for the inferior vena cava, better image quality and vessel definition, and more stable venous signal over time compared to gadofosveset trisodium. There were no adverse reactions to either contrast agent. Ferumoxytol was concluded to offer a wider time window for optimal image acquisition and a near-ideal approach to high-resolution venography in children.
